Accessible Space Inc. (ASI) is a national nonprofit organization providing housing and personal care services to individuals with physical disabilities, seniors and veterans.

Accessible Space, Inc. is currently seeking a Full Time Case Manager to join our team at NCEP - Spencer Street Campus! 

 

Our Spencer Street campus is located at 4148 Spencer Street in sunny Las Vegas NV, 89119. 

 

Our Case Manager provides case management services in order to facilitate a smooth admission; ongoing individual services/treatment and an appropriate and viable discharge plan for clients. 

 

In addition, you will conduct a thorough admission for new clients by providing program explanation, treatment, and schedule and program hours.  Provide clients and families with an initial orientation and tour of the facility; provide and explain clients rights and responsibilities to clients and families at admission and ongoing through the program as needed.

 

Other responsibilities include: Coordinate initial, progress, and discharge summary assessments of NCEP clients in all applicable clinical areas of physical function, cognitive skills, and independent living, social interaction with members of the clinical team. Must be able to present assessments to clients, families, payers, and other appropriate parties

Must haves: Bachelor's Degree in social work, psychology, sociology, or a closely related field.

 

A minimum of three years experience as a case manager (or closely related position) in a private or public rehabilitation center, healthcare setting, or social service environment.  

 

Prior experience working in rehabilitation program, day treatment, and/or residential programs for individuals with traumatic brain injuries.
